Please complete this form and send it with your amended 1996 return. If you need more time to complete your Form 8395-L, Paperwork Reduction Act Notice Receipt Acknowledgment, you may file Form 109845-P, Extension of Time to File Paperwork Reduction Act Notice Receipt Acknowledgment. Please provide all relevant documentation to demonstrate the need for an extension.

    I am filling out Form 109845-P, Extension of Time to File Paperwork Reduction Act Notice Receipt Acknowledgment and I don’t understand Section III, Subsection 2, line 6. It says that I must take the date of line 2 from Section IV, Subsection 3.2 of prior submitted Form 23012-X, Anticipated Delay to File Extension of Time to File Paperwork Reduction Act Notice Receipt Acknowledgement. However I failed to anticipate the delay and never filed that form! So what am I to do?
